Please fill out a form for each child. In order for your child to attend we will need this form filled out prior to our VBS beginning on July 24. We need to plan ahead for seating and such. Visitors are welcome during the week. If your visitor is coming the first day please fill out a form and submit it for them. This form will also be available if you are going to bring a visitor during the week. You can fill out a form the night before and submit it.
